Rush Creek Bible Church Student Participant Release 2023-2024
We care about your students’ well-being at Rush Creek Bible Church and are privileged to have you entrust them to our care. We aim for meaning and purpose while having fun, yet also making their safety a priority. In light of this, we understand the risk that accidents may arise and we have measures in place to get the appropriate help. 

With this release form, would you support us as we work to protect and help your students during their times of ministry in and through Rush Creek? 

Injury and Transportation Release
I will not hold Rush Creek, its staff, or any participating organization responsible for accidents that might occur while our child participates in Rush Creek Bible Church programs. In consideration for the opportunity to participate in the activity, the participant (or the participant’s parents or guardians, if the participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the activity. If an accident does occur, I give the event staff approval/permission to take my child to the hospital to receive medical attention until we can arrive. The parent or guardian of each child is responsible for costs associated with necessary medical attention. As such, you as a parent or guardian assume the risk of injury or other harm and also release Rush Creek Bible Church from liability for injury, illness, or other issues resulting from or in any way related to your child’s participation. 

Understanding the Risk

​​As parent/guardian of the above-named student, I give my permission for my student to participate in the above-named activities. I give permission for my child to be transported by approved drivers of Rush Creek Bible Church. I also acknowledge that if my student has to return home early for discipline violations, it will be at my expense.
We aim to have fun with your students. Part of making memories is capturing those times through pictures and videos! By submitting this form, you acknowledge that your child’s photo may be shared as we tell others about the ministry happening in and through Rush Creek.
